What is the mechanism of action of Avanafil?
Avanafil is a Phosphodiesterase enzyme inhibitor having 100 times more selectivity towards Phosphodiesterase type 5 (PDE 5) than other Phosphodiesterase types. Phosphodiesterase type 5 (PDE5) is an enzyme which is responsible for the breakdown of cGMP in the penile tissue (corpus cavernosum). During sexual stimulation, nerve endings and endothelial cells release nitric oxide which triggers the cGMP production in smooth muscle cells of the penis. cGMP dilates penile blood vessels resulting in improved blood flow to the penis causing erection. Avanafil inhibits PDE-5, thereby enhancing the concentration of cGMP and thus it improves the erectile dysfunction.
What are the indication of Avanafil?
Avanafil is a recently approved medicine for erectile dysfunction in males due to one or more causes. It is the safest and faster acting drug among other Phosphodiesterase inhibitors.
What are the doses of Avanafil?
The dose can vary from 50 mg to 200 mg depending upon the tolerance, age, medical condition of the patient. It has to be taken orally with or without food. It acts effectively only when consumed 30 minutes preceding physical intercourse. Start with a smallest effective dose. Do not take more than 200 mg of Avanafil a day. No two doses of Avanafil should be repeated within 24 hours.
What to do in case of missing a dose?
This medicine is indicated to be used only when required so there is no chance of missed dose.
What are the side effects associated with Avanafil?
Avanafil is a safe drug but some side effects like headache, pain in back, flushing, blocked nose and dizziness can occur. These are not serious and subside with time. But if they prolong or intensify then consult your doctor.
However few serious side effects are observed in less than 1% of male population taking Avanafil. These are:
- Whole body — swelling of hands of feet, tiredness
- Cardiovascular system — chest pain (angina), rapid and irregular heart beat
- Digestive system — abdominal discomfort, low or high blood glucose levels, vomiting
- Musculoskeletal system— myalgia (pain in muscles), muscle spasms, musculoskeletal pain
- Nervous system — depression, lack of sleep (insomnia), giddiness.
- Respiratory system — cough, troubled breathing (apnoea), wheezing, epistaxis (bleeding from nose)
- Skin – itching, redness, irritation
- Urinary system– blood in urine, kidney stones, urinary tract infection
- Reproductive system- Priapism (painful erection lasting for more than 4 hours)
What are the contraindications with the use of Avanafil?
- Use of nitrates with Avanafil is highly contraindicated. It can cause severe lowering of blood pressure and fainting.
- Individuals having allergy to Avanafil or any other ingredient of the tablet should not administer Avanafil.
- Female use of Avanafil is prohibited.
- Use of Alcohol is to be avoided with Avanafil or any other PDE5 inhibitors because it can lower the blood pressure severely.
What precautionary measures should be followed while using Avanafil?
- Always follow the dosage regimen prescribed by the doctor, do not alter the dose without consulting your doctor.
- Avoid using nitrates with Avanafil. In case if medically situation demands then take the dose of nitrate after at least 12 hours of taking Avanafil.
- Start with 50 mg of Avanafil, if you are already taking alpha blockers.
- Two doses of Avanafil should not be taken within 24 hours.
- Do not administer other Phosphodiesterase inhibitors concomitantly with Avanafil.
What to do in case of over dosage with Avanafil?
If you have taken high doses of Avanafil (dose of 800 mg or more) immediately seek medical attention.
What are the drug interactions with Avanafil?
If you are using different medicines at the same time they can cause severe reactions or produce unwanted effects. Prepare a list of all medication you are using and discuss with your doctor.
Medicines that interact with Avanafil are:
- Nitrates like nitro glycerine, isosorbide dinitrate, isosorbide mononitrate.
- Alpha-Blockers like terazosin, prazocin, doxazocin.
- CYP3A4 Inhibitors like saquinavir, telithromycin, itraconazole, ketoconazole, ritonavir, clarithromycin, indinavir and nelfinavir.
- HIV protease inhibitors like ritonavir.
- Medicines for high blood pressure like amlodipine, enalapril.
- Other ED medicines belonging to category of Phosphodiesterase inhibitors (PDE5) like sildenafil, tadalafil, verdenafil.
